Job Description

This is a per diem SLP position working weekdays and/or weekends, 8am-4pm at Northwest Transitions Acute Inpatient Rehabilitation Facility located at 2025 W Orange Grove Road. Specific help needed every Friday and to cover vacations.

Seeking a candidate who is dedicated to quality patient care, working in an inpatient rehabilitation setting. Join a collaborative rehabilitation department committed to an interdisciplinary team-based approach to medicine. Work in a new 30 bed inpatient facility that specializes in getting patient home and back to the community. Our rehab facility has been recognize as a top 10% facility for 3 years.

 

Summary:

Diagnose and treat speech, language, cognitive-communication, voice and swallowing disorders within the inpatient rehab setting. Appropriately recommends instrumental swallowing evaluations for MBS and FEES. Develop, carry out, and document treatment programs. Provide education and training for patients, caregivers, and families. Coordinate within the interdisciplinary team including (but not limited to) nursing, physicians, specialists, respiratory therapy, dieticians, case management, PT/OT,  etc. Consider, plan, and coordinate ongoing treatment plans to facilitate safe and effective discharge. 

 

Position Qualifications/Minimal Requirements:

Master's degree in speech-language pathology and Arizona state license required. Certificate of clinical competence in speech-language pathology (CCC) preferred.  BLS certification required.

Experience:

1 Year of Rehab or SNF experience preferred.
