This is a Full-Time Floating Case Management position to provide coverage for Oro Valley Hospital, the Oro Valley Skilled Nursing Unit and Northwest Transitions Rehabilitation.
Summary:
Oro Valley Hospital Case Management is seeking an experienced Medical Social Worker with Acute hospital experience. Join a group of helpful individuals - who want you to be successful. Find camaraderie in daily huddles and collaborate among teams.
The schedule for training will be Monday through Friday 9am to 6pm for a 6 week period. Once trained and comfortable in the position, the schedule will be flexible based on census with an expectation of 2-3 shifts per week, including one weekend day. We are looking for a social worker that has discharge planning experience in a medical setting. The candidate will need to be able to work independently and as a part of the IDT team.
Position Purpose:
The Case Manager will be primarily responsible for discharge planning which may include, assessments, psychosocial evaluations, coordination with post-acute service providers, patient and family support, care conferences, and inter-disciplinary team meetings. The candidate must be flexible as this position will cover discharge planning at one of three facilities (Oro Valley Hospital, the Oro Valley Skilled Nursing Unit and Northwest Transitions Rehabilitation).
Provides assessment and crisis intervention in acute care settings; plans, organizes, and implements social work services for patients and families. Provides long term care planning for patients/families. Responsible for complex discharge planning. Intervenes as needed with hospital patients with mental health problems or substance abuse problems. Provides assessment with case management for pediatric patients.
Position Qualifications/Minimum Requirements
Education:
Required: Masters of Social Work degree
Experience:
Required: Two (2) years acute clinical experience
Skills:
Able to handle crisis situations that require judgment critical to patient welfare; Appropriate prioritization and planning skills, while achieving safety and quality in a high volume environment; Engaging and effective communication and service skills; Adherence to policy and procedures; Process improvement oriented; Self-initiative to assist with whatever is needed.
