This position is Per Diem (as needed) working at Houghton Hospital.

Houghton Hospital Case Management is seeking an experienced Social Worker with Acute hospital experience.

Position Purpose

Provides assessment and crisis intervention; plans, organizes, and implements social work services for patients and families. Provides long term care planning for patients/families. Responsible for complex discharge planning. Intervenes as needed with hospital patients with mental health problems or substance abuse problems. Provides assessment with case management for pediatric patients.

Position Qualification/Minimum Requirements

Education:

Required: Bachelor of Social Work or related degree such as Counseling or Psychology.

Experience

Required: Two (2) years of clinical/hospital experience 

Skills: 

Able to handle crisis situations that require judgment critical to patient welfare; Appropriate prioritization and planning skills, while achieving safety and quality in a high volume environment; Engaging and effective communication and service skills; Adherence to policy and procedures; Process improvement oriented; Self-initiative to assist with whatever is needed.

Licenses/Certifications:

For Skilled Nursing Unit: Fingerprint Clearance Card

Required: L-AZ-Social Worker LCSW
