Job Description

This is a Per Diem (as needed), day shift, position working as an Admissions Clinician in the Inpatient Rehab Unit. 

Assists with placement of appropriate patients to the rehab unit by responding to and carrying out pre-admission screenings for patients referred to the rehab program along with development and implementation of a referral program for patients, families and the community.

Serve as an admission liaison for Northwest Transitions. Reviews admitting department operations. Ensures compliance with applicable standards. Oversees the inpatient functions, bed assignments, and completion of preliminary paperwork for entering patients. Works with medical, nursing, and therapy staff to ensure appropriate patient placement. Confirms that all insurance benefits coverage meets standards of admission as dictated by policy.

Position Qualifications/Minimum Requirements

Education: Required: ADN or Diploma in Nursing Program
-OR- Bachelor/Master/Doctorate degree from an accredited program in Occupational Therapy
-OR- Bachelor/Master/Doctorate degree from an accredited program in Physical Therapy 

Experience: Required: Two years clinical experience in rehab setting

Skills: Ability to comprehensively review a medical record for rehab medical necessity; consistently applied organizational skills; results oriented; independently goal directed; able to multi-task; able to communicate well with many diverse audiences - professional and lay persons; able to meet established time frames; excellent customer service skills with both internal and external customers; able to work independently; experience with computer and other electronic data entry devices.

Licenses/Certifications: Required: Current Arizona or compact state RN license
-OR- Current Arizona Occupational Therapy license
-OR- Current Arizona Physical Therapy license
