Job Description

This is a per diem (as needed) position working weekdays and/or weekends, 8am-4:30pm at Northwest Transitions Rehabilitation facility located at 2025 W. Orange Grove Road.

Join the Acute Inpatient Rehabilitation Team at Northwest Transitions Rehabilitation facility. The facility is a new 30 bed inpatient rehab with all private rooms. The rehab team is collaborative and interdisciplinary as well as recognized in the top 10% of the nation for 3 consecutive years. The Transitions building is located on the Northwest Medical Center Campus.

 

Summary:

Evaluating and assessing needs of patients and formulating treatment plans. Provides therapy services defined in treatment plans. Works cooperatively with physicians, case managers, and other healthcare staff. May supervise physical therapy assistants, aides, and athletic trainers. Relies on experience and judgment to plan and accomplish goals. Performs a variety of tasks. A certain degree of creativity and latitude is required. Typically reports to a manager. Plans and implements specific treatment programs for individual patients according to the principles and practices of physical therapy. Collaborates with interdisciplinary treatment team.

 

Position Qualifications/Minimum Requirements

Education:

Required: Bachelor/Master/or Doctorate degree from an accredited program in physical therapy.

Experience:

Preferred: One (1) year in acute care

Skills:

Appropriate prioritization and planning skills, while achieving safety and quality in a high volume environment; Engaging and effective communication and service skills; Adherence to policy and procedures; Process improvement oriented; Self-initiative to assist with whatever is needed.

Licenses/Certifications:

Required: Current Arizona Physical Therapist license; American Heart Association (AHA) or American Red Cross(ARC) or American Safety and Health Institute (ASHI) BLS certification. Skilled Nursing Unit: Fingerprint Clearance Card
