MDS Assessment Coordinator - RN/LPN
Job Description
Seeking an experienced MDS Coordinator for a Full time day shift position in our 33 Bed Skilled Nursing Unit at Oro Valley Hospital.
Position Purpose
The MDS Assessment Coordinator is accountable for coordination and completion of the MDS and interdisciplinary care plan on all patients admitted to the Transitional Care Unit. Is responsible for completing the Resident Assessments, the recommendation, development and implementation of Resident Care Plans, and for keeping the Director of Nursing /Administrator informed of both progress and potential problems. Maintains operational aspects of the patient care unit in collaboration with the Director of Nursing.
Position Qualifications/Minimum Requirements
Education: Required: Associates or Diploma in nursing program Preferred: Bachelors degree in nursing
Experience: Required: 1 year experience in Long-term healthcare
Skills: Ability to provide daily operational leadership and coordination for relevant teams. Appropriate prioritization while achieving safety and quality in a high volume environment; Engaging and effective communication and service skills; Adherence to policy and procedures; Process improvement oriented; Self-initiative to assist with whatever is needed.
Licenses/Certifications: "Required: Current Arizona RN license or compact state license OR current Arizona LPN license. Certifications fromAmerican Heart Association (AHA) or American Red Cross(ARC) or American Safety and Health Institute (ASHI) for BLS
