Manager of Case Management
This is a Full time salaried position working in the Social Service department at Northwest Medical Center.
Oversees a staff of case managers responsible for patient care coordination. Develops and implements case management programs, including utilization review, intake or discharge planning, and managed care contracting or negotiation. Evaluates patient care data to ensure that care is provided in accordance with clinical guidelines and organizational standards. Seeks treatments that balance clinical and financial concerns with the family's needs and the patient's quality of life.
Required: Associates or Diploma in nursing program, or Masters degree in Social Work (MSW)
Preferred: For RN incumbents, BSN or MSN
Required: Five (5) years experience in case management, and Two (2) years of supervisory experience.
Required: Current AZ RN Licensure or compact state RN license or LMSW license.
Preferred: Case Management or Utilization Management certification.
