Job Description

This is a full-time position, 40 hours per week with a weekend rotation. Our free standing Inpatient Rehab facility is located at 2025 W Orange Grove Rd, Tucson, AZ 85741

Work with case management, nurses, physicians, therapists, and rehab director to admit patients to the Northwest Transitions new Acute Inpatient Rehabilitation Unit.   Serve as a part of the discharge planning team, assessing patients for the appropriate level of care at discharge from Northwest Medical Center and Oro Valley hospital.  Serve as a part of the overall admissions team, including other liaisons, the rehab director, therapy and nursing supervisors, and rehab physicians.  Communicate with physician and charge nurses regarding special considerations for new admissions to ensure continuity of care.  Educate patients and family members regarding post acute services and appropriate levels of care. 

Serve as an admission liaison for Northwest Healthcare Acute Inpatient Rehab unit. Reviews admitting department operations. Ensures compliance with applicable standards. Oversees the in-patient functions, bed assignments, and completion of preliminary paperwork for entering patients. Works with medical, nursing, and therapy staff to ensure appropriate patient placement. Confirms that all insurance benefits coverage meets standards of admission as dictated by policy. The rehab unit is part of Oro Valley Hospital and located on the campus of Northwest Medical Center.

Required Education and Licensure: 

Associate's in Nursing OR Bachelor's in Occupational Therapy/Physical Therapy 

Arizona or compact state nursing license OR Arizona Occupational/Physical Therapy license
