Clinical Care Coordinator
Job Summary
The Clinical Care Coordinator ensures the delivery of high-quality, patient-centered care by coordinating and monitoring clinical operations within the unit or department. This role collaborates with the management team to direct nursing care, support staff, and promote a culture of excellence aligned with the organization's mission and values. The Clinical Care Coordinator acts as a resource for staff, assists with operational functions, and fosters teamwork to enhance patient outcomes and departmental performance.
Essential Functions
- Assists with admission and discharge processes, ensuring efficient patient flow and high-quality care.
- Continuously evaluates the quality of patient care services provided in the unit, collaborating with the management team to implement improvements.
- Collaborates with subject matter experts and managers to foster a teamwork-oriented environment that supports improved patient outcomes and service delivery.
- Promotes a patient-first philosophy, engaging in service recovery efforts when necessary to address concerns and enhance patient satisfaction.
- Supports employee engagement initiatives by operationalizing strategies such as employee rounding, hourly patient rounds, and other best practices.
- Provides input into the hiring, training, and development of new staff, recommending actions to address performance issues and ensure policy compliance.
- Assists with staff scheduling and manages labor resources according to patient care needs and established productivity guidelines.
- Develops and maintains an organizational structure to facilitate the safe, effective, and efficient delivery of patient care.
- Prepares weekly and monthly nursing schedules and implements cost-control measures to ensure budgetary compliance.
- Participates in infection control surveillance, environmental hazard monitoring, and emergency preparedness drills to ensure safety and compliance.
- Serves as a resource for nursing staff, addressing clinical and operational questions, and providing mentorship and guidance.
- Performs other duties as assigned.
- Maintains regular and reliable attendance.
- Complies with all policies and standards.
Qualifications
- 2-4 years of clinical nursing experience required
- 1-2 years of experience as a charge nurse or clinical care coordination required
Knowledge, Skills and Abilities
- Strong clinical knowledge and expertise in patient care delivery.
- Effective leadership and interpersonal skills to guide and support nursing staff.
- Proficiency in coordinating patient care and managing clinical operations.
- Ability to prioritize and manage multiple tasks in a fast-paced environment.
- Strong communication skills to foster collaboration among interdisciplinary teams.
- Knowledge of regulatory and accreditation standards applicable to nursing services.
- Problem-solving and decision-making skills to address clinical and operational challenges.
Licenses and Certifications
- RN - Registered Nurse - State Licensure and/or Compact State Licensure required
- BCLS - Basic Life Support required
- ACLS - Advanced Cardiac Life Support required
- PALS - Pediatric Advanced Life Support if supporting Pediatric ICU required
