Job Description

This is a full time position working a day shift.

Work with case management, nurses, physicians, therapists, and rehab director to admit patients to the Northwest Transitions new Acute Inpatient Rehabilitation Unit.   Serve as a part of the discharge planning team, assessing patients for the appropriate level of care at discharge from Northwest Medical Center and Oro Valley hospital.  Serve as a part of the overall admissions team, including other liaisons, the rehab director, therapy and nursing supervisors, and rehab physicians.  Communicate with physician and charge nurses regarding special considerations for new admissions to ensure continuity of care.  Educate patients and family members regarding post acute services and appropriate levels of care. This position requires 1-2 weekends per month and is an 8am-5pm position.
